Required Enrollment Forms
- All required documents must be uploaded directly to the online enrollment application.
- Make sure you all ALL necessary documents before starting the online enrollment application.
Required documents for all students:
- Original Birth Certificate
- Immunization Records
- Proof of Residency (Electric or Gas Bill)
- Parent ID
- TK & Kindergarten:

- 6th-8th Grade:
- Transcripts or current report card
- Behavior Report
TK and Kindergarten please see the additional REQUIRED forms below. These forms must be filled out and uploaded to the online enrollment application. Both the Health Exam/Physical and Oral Health Form must be signed by a physician.

Scanned documents should only capture the document and not the background surrounding the document. All scanned documents must be clear to read.
